Agreement Form

The agreement form is the form completed by someone applying to the ILF (or a person on their behalf) once they have had their assessment and received an offer of funding.  Completion and return of the form means that the applicant (or their representative) understands the purpose of the ILF award, how it was calculated, how it can be used and their responsibilities as someone receiving money from the ILF.  Payment of the ILF award usually starts when the agreement form is received.

Although the agreement form is available to look at on the website we cannot accept a copy that you have printed off yourself.  You must complete an original agreement form.  If you make a mistake filling it in or mislay the original, please phone us and we will send you another one.
