1. The information we collect and how we use it
We will use information about you (whether obtained via this website or otherwise) as described below and to respond to any queries raised by you in any e-mails to us.
When you send us an application form we need to know your name, date of birth, postal address, e-mail address, telephone number, fax number, National Insurance Number. We also need to know contact details for your social worker or local authority representative. They will confirm whether you meet the criteria, and make a declaration concerning the value of services they are willing to provide you with as part of any proposed joint care package.
We gather this and other information to allow us to process your application and verify your eligibility, and, if you are accepted as a client of The Independent Living Funds, to administer payment to you, to check your continuing eligibility to receive payments, to monitor use of the payments made to you where we consider this necessary, and to enforce our rights. The relevant information will be used by us, our agents and sub-contractors to communicate with you on any matter relating to your application and (if you are accepted as a client) to the provision of payments and to provide you with information relating to such payments, along with forms that you will need to complete and return before payments can commence. These forms ask for details of the bank account you would like the money to be paid into, and details of the care you have employed. We will also use your information as indicated on our application form when you sign this.
We may also use information about you as specified on other documentation which you are asked to sign from time to time.
We may also use aggregate information and statistics for the purposes of monitoring website usage in order to help us develop the website and our services and may provide such aggregate information to third parties. These statistics will not include information that can be used to identify any individual.
If you e-mail a query to us we reserve the right to respond to you by e-mail.
If you supply information to us about other people we process it on the basis that you have obtained the consent of the relevant person for us to do so in connection with your application and (if your application is successful) for the other purposes set out in this policy. Please ensure that such consents have been obtained.

If the Independent Living Funds is in the future integrated with another organisation or body your details may be disclosed to our advisers and any such organisation or body and their advisers and may be processed by any such new organisation or body for the purposes set out above.
